Spotlight: The Spectrum Special Issue on Malnutrition

A special issue capturing first-hand and/or evidence-based advice on addressing malnutrition in older adults

In October 2018, the Academy of Nutrition and Dietetics’ Healthy Aging Dietetic Practice Group released a special issue of The Spectrum, its peer-reviewed publication, on malnutrition and older adults. The edition’s articles provide first-hand experiences and evidence-based guidance on addressing malnutrition in older adults across the care continuum. The content shared in the special edition is based on the 2017 resource, the National Blueprint: Achieving Quality Malnutrition Care for Older Adults, published by the Defeat Malnutrition Today coalition in partnership with Avalere Health. The Blueprint aimed to highlight the increasingly alarming crisis of older adult malnutrition in the United States, sharing pertinent background statistics and offering best practices to be adopted in various acute-, post-acute, and community care settings.

Using the Blueprint as the standard for optimal malnutrition care in older adults, the special edition includes information about meal delivery programs, contributing factors to older adult malnutrition (including transitions of care across environments), ways to reduce barriers to appropriate care, malnutrition training information, malnutrition screening tools, and successful partnerships. Contributing authors are experts in the fields of nutrition, physical therapy, nursing, community health, and more.

Be sure to check out the Maryland Malnutrition Model article on page 42, where the authors use MQii as a reference in their own state-wide care planning post-discharge project that includes an evidence-based pathway, clear outcome metrics, and billing processes to allow for sustainable community-based care for malnourished individuals. Many participants are working with the MQii Team to establish programs focused on malnutrition care beyond the hospital. MQii at FNCE

The Academy of Nutrition and Dietetics’ hosted its annual Food and Nutrition Conference and Expo (FNCE) in Washington, D.C. from October 20-23, and prominently featured MQii (as well as other key malnutrition topics) throughout the event. MQii Learning Collaborative members participated in various sessions and had the opportunity to network with various industry leaders, including registered dietitian nutritionists (RDNs), nutrition science researchers, policy makers, and healthcare providers. Below are some of the MQii highlights from the event:

  • MQii Video at FNCEDuring the Opening Session, the Academy featured a video highlighting the MQii and the benefits of malnutrition quality improvement. Two Learning Collaborative participating sites, the University of North Carolina Medical Center and Intermountain, were featured in the video, sharing their experiences with the initiative. If you did not have a chance to attend the opening session, check out the video to hear key insights and takeaways from these two sites.

  • MQii Symposium: “Dietitians Leading Innovation: Using Data for Quality Improvement and Patient-Focused Transitions of Care”: On Monday, October 22nd,three MQii Learning Collaborative leaders – Kelly Danis, RD, LDN, FAND at the University of Pittsburgh Medical Center, Sadeq Quraishi, MD, MHA, MMSc at Massachusetts General Hospital, and Wendy Phillips, MS, RD, CNSC, CLE, FAND at Morrison – participated in a session to share how RDNs are using data to show the value of nutrition care and highlight actionable steps to partner with information technology colleagues to collect malnutrition quality improvement data. The session also provided examples of opportunities to better address patients’ nutrition needs throughout transitions of care.

  • Learning Collaborative “Gab, Grub, and Graffiti” Reception: On Sunday, October 21st, MQii Learning Collaborative participants gathered to celebrate MQii accomplishments, recognize Learning Collaborative hospital achievements, and network with one another as well as MQii Leaders. Participants were posed four specific questions about MQii, with responses captured by an artist contracted to provide “graphic facilitation” at the event. 

  • “The Future of Pediatric Malnutrition: Establishing Prevalence, Ensuring Prevention”On Sunday, October 21st, attendees had the opportunity to hear Patricia Becker, Pediatric Nutrition Specialist at Dayton Children’s Hospital and Molly Vega, MS, RDN, LD, Pediatric Clinical Dietitian at Texas Children’s Hospital discuss the application of MQii principles to pediatric populations. The session highlighted ways that MQii best practice recommendations and tools can be adopted among pediatric dietitians.
  • Poster Session Highlight: On Sunday, October 21st, Amanda Goldman from Catholic Health Initiatives shared a poster on “The Impact of Nutrition-Focused Quality Improvement Interventions on Length of Stay and Readmission Rates among Hospitalized Malnourished Patients.” Attendees had the opportunity to ask questions regarding best practices to improve patient outcomes through better nutrition care based on the findings from the poster.

  • “Strategies to Improve Care Transition to Reduce Malnutrition”: On Tuesday, October 23rd, participants heard from experts in the field on successful approaches and innovative ideas for RDNs to “stop the gap” in patient care continuity between acute and post-acute care settings. Kris Mogensen, MS, RD-AP, LDN, CNSC, from the Brigham and Women’s Hospital and Julie Grim, MPH, RD, LD, from the University of Rhode Island, were the key speakers of the session. Throughout the session, the speakers highlighted various resources, including the Dialogue Proceedings report prepared by Avalere Health, the Academy, and Defeat Malnutrition Today in July 2018, as a tool for interested stakeholders to use in advancing patient-centered malnutrition care transitions.

Disseminate Your Project

Do you have a great story to tell about your organization’s malnutrition quality improvement efforts to date? For example:

  • Did you overcome significant hurdles to pull together an interdisciplinary team?
  • Do you have an interesting story regarding how you engaged with your IT department to collect and review your performance data?
  • Do you have results to share regarding how the quality of your care changed following your pilot project, or its effect on patient outcomes?
  • Have you made strides in considering ways to support malnourished patients as they transition across care settings (e.g., pre-admission or post-discharge programs)?
  • Have you found interesting or meaningful ways to better engage patients in their malnutrition care?
  • Even if you are just getting started, do you have insights on what tools/resources/information were most useful to initiate your quality improvement project?

If so, we want to help you share your experience! We are exploring opportunities to highlight and disseminate MQii Learning Collaborative members’ efforts to date. If you are interested in participating in this effort, please consider developing a brief abstract to reflect your story and key insights or findings from your efforts. Please send it to your MQii Point of Contact by December 21, 2018. We will then work with you to determine how best to disseminate your work.

Expert Webinar: Opportunities to Advance for Care Transitions and Coordination for Malnourished Hospitalized Adults

Join us on Tuesday, November 13th at 3:00 PM ET for the MQii Expert Webinar, “Opportunities to Advance Care Transitions and Coordination for Malnourished Hospitalized Adults.” Participants will have the opportunity to hear from experts in the field regarding opportunities to better support patients in continuing their nutrition care and meeting their nutrition needs as they leave the hospital setting. The session will:

  • Highlight best practices, tips, and insights for communication and handoffs from the hospital to the next-in-line provider (e.g., rehabilitation facility, long-term care facility, skilled nursing facility, community care provider);
  • Review opportunities for patient engagement and counseling regarding nutrition during care transitions; and
  • Identify opportunities to connect patients to services or benefits that could support their nutrition needs post-hospitalization.

Expert Webinar: Engaging the Patient in Nutrition Care Planning in the Hospital and Beyond

Join us on Friday, November 30th at 12:00 PM ET for the MQii Expert Webinar, “Engaging the Patient in Nutrition Care Planning in the Hospital and Beyond.” Participants will have the opportunity to hear from experts in the field implementing patient-centered care practices in malnutrition. Attendees will also have the opportunity to ask questions and troubleshoot any challenges regarding best practices for malnutrition-focused patient engagement and shared decision making. Following the session, attendees will:

  • Understand the role of patients and caregivers in contributing to nutrition care decisions;
  • Have greater knowledge of ways to engage the patient and caregiver in care planning and shared decision making; and
  • Be familiar with tools and resources to support improved patient and caregiver engagement while in the hospital and during discharge.

The webinar will feature Karim Godamunne, MD, Chief Medical Officer of the North Fulton Hospital in Atlanta and Grace Whiting, JD, President and Chief Executive Officer at the National Alliance for Caregiving.

November EHR-Specific Coffee Breaks

In the month of November, we will be offering two Coffee Breaks geared toward specific EHR systems. The two most commonly implemented EHR platforms in the Learning Collaborative are EPIC and Cerner systems. We will be sharing insights and recommendations for implementing components and capturing data across the malnutrition care workflow from screening to discharge within specific EHR platforms.

Please note, if you are not an EPIC or Cerner site but would like to hold a group discussion around your EHR platform, please email the MQii Team and describe what EHR system you employ and any current or projected challenges surrounding said implementation. We will gather those responses to form smaller subgroups for any of the other platforms where interest is identified.


Special Webinar: Addressing Malnutrition within the Veterans Administration (VA) Hospitals and Beyond: Lessons and Opportunities for non-VA Organizations

Join us on Thursday, December 6th at 1:00PM ET for a special VA-focused webinar. The VA has long been focused on ensuring it meets the nutrition needs of its population and has undertaken significant efforts in recent years to support better malnutrition care, both within and beyond the hospital. Attendees will have the chance to learn from the VA’s experience and pose questions regarding how to apply these activities to non-VA hospitals. The session will:

  • Share best practices from the VA’s efforts to improve the quality of nutrition care for veterans;
  • Highlight the ways in which the VA is supporting veterans’ nutrition needs throughout the healthcare continuum; and
  • Outline lessons learned and tips for non-VA hospitals seeking to implement similar quality improvement activities.

Upcoming Conferences and Events

The Gerontological Society of America (GSA) will host its Annual Scientific Meeting from November 14-18, 2018 in Boston, Massachusetts. GSA is the oldest and largest interdisciplinary organization devoted to research, education, and practice in the field of aging. Join Mujahed Khan, Senior Manager, Quality Improvement at the Academy of Nutrition and Dietetics and Meredith Ponder, Policy Director for Defeat Malnutrition Today, as they share a poster on “Advancing Patient-Centered Malnutrition Care Transitions” on Thursday, November 15, from 1:00 – 3:00 PM to highlight multi-stakeholder insights and recommendations for improving transitions for malnourished patients across acute, post-acute, and community settings. If you will be presenting at the GSA Annual Scientific Meeting, please let us know so we can share your presentation information with other Learning Collaborative members!

 

The Institute for Healthcare Improvement (IHI) will host its 30th National Forum from December 9-12, 2018 in Orlando, Florida. IHI is a leading innovator, partner, and driver of results in health and healthcare improvement worldwide. The National Forum aims to convene various stakeholders to explore how improvement science methodologies can be used to effect real change in patient safety and care. On Tuesday, December 11 from 4:40 – 6:30 PM, join Kelsey Jones, Senior Director at Avalere Health, as she presents a poster on behalf of MQii on opportunities to scale existing models being implemented in the U.S. to better address poor nutrition as a social determinant of health across different care settings. Please reach out to us if you will similarly be presenting at the IHI National Forum so we can share information about your presentation with MQii Learning Collaborative participants!
